Selbst kleine Veränderungen des HGH-Spiegels haben Auswirkungen auf den Körper. Eine im Jahr 2023 publizierte Metaanalyse von Mameli et al. fand keine signifikante Verbesserung der Therapieadhärenz unter LAGH im Vergleich zu hGH [19].
Im selben Jahr wurde rekombinant humanes GH (rhGH) für die Therapie in den USA verfügbar. Aus technischen Gründen konnte zunächst nicht das 191 Aminosäuren (AS) lange authentische hGH hergestellt werden, sondern das, um eine AS längere Met-r-hGH (Somatrem). Ein rekombinantes authentisches rhGH (Somatropin) wurde erstmals 1987 in Europa für GHD zugelassen. Mit rhGH werden heute nach entsprechenden Studien nicht nur Kinder und Erwachsene mit GHD im Sinne einer Substitution, sondern auch Kinder mit anderen Kleinwuchsformen z. Lebensjahr folgt eine Reduktion um weitere rund 10 Prozent pro Altersdekade mit etwas langsamerem Absinken bis zum 60. IGF-I ist als anti-apoptotischer Wachstumsfaktor wesentlich an der Regulation von Proliferation einerseits und programmiertem Zelltod zum Wohl des Gesamtorganismus andererseits beteiligt.
